• Physicals Information

     

    Each athlete must complete a physical each year in order to be eligible to play a sport at Cedar Crest. Physicals can be completed by a licensed doctor using this CIPPE form and MUST be completed after June 1st of the year for participation in the sport. Physical forms dated earlier than June 1st will not be accepted. All sections must be completed and should be turned into the Athletic Trainers at least two weeks prior to the start of the season. Athletes are not permitted to practice until the required paperwork is submitted. Physicals may be used for multiple sports within the same school year.     If your son or daughter has suffered an injury or illness during school PE, sports, or out of school since the completion of their physical and wants to participate in a sport in an additional season each year, a CIPPE section 8 must be completed to be eligible to try out. If you have seen a doctor, whether on your own accord (NOT RELATED TO SCHOOL), by the nurse, or athletic trainer, this form must be completed.

    The Athletic Training Staff now has the ability to accept the PIAA forms electronically. Please use the information in the Athletic Training Software Handout to learn how to use this software to submit forms electronically and view forms that need to be completed.

    In addition, if your son or daughter requires any medications to be given at school, please fill this Medication Consent Form out and have it signed by both you as the parent/guardian and a doctor.

     


    Physicals offered through Cedar Crest High School

    Physicals are offered at Cedar Crest High School twice each summer. Below are the dates physicals will be offered to students to be eligible for the 2019-2020 school year:

    • Thursday June 6th, 2019 2-6pm
    • Thursday August 1st, 2019 2-6pm

    All physicals take place in the Cedar Crest High School Gym B. Time slots for the physicals are listed below:

    • 2:00 - 3:20pm: 11th and 12th Grade Students
    • 3:20 - 4:40pm: 9th and 10th Grade Students
    • 4:40 - 6:00pm: 7th and 8th Grade Students

    All paperwork for the Winter 2019 season must be turned in by November 11th to guarantee that it will be processed in time for the first day of the season.

    Dr. Yocum will be available on November 14th for recertification from an injury or illness suffered during the fall season.
