Each athlete must complete a physical to be able to play a sport in the Cornwall-Lebanon School District. The Cornwall-Lebanon School District Athletic Trainers are implementing new procedures for the 2020-2021 school year in regards to physicals. Please follow the instructions below:

Physicals will be hosted at Cedar Crest High School on the following dates:

  • July 30th, 2020 from 3-6pm
  • August 4th, 2020 from 6-9pm
  • August 6th, 2020 from 3-6pm

All physicals will be by appointment only – There will be no walk-ins accepted on the day of physicals. Please schedule your appointment today!

The process of completing the physical will be different this year – ALL PHYSICALS (Sections 1-5) WILL BE TURNED IN ELECTRONICALLY. Instead of collecting the entire PIAA CIPPE Packet in paper form, the Athletic Training System (ATS) website will be used. Parents and athletes will need to complete sections 1-5 and the Risk of Injury Acknowledgment in the ATS website prior to arriving for the scheduled appointment for the physical. Please refer to the ATS website instructions to get started. Section 6 is the portion completed by the doctor. This form will be completed on paper and turned into the Athletic Training staff after the physical.

Screening for COVID-19 symptoms will be completed before entering the building. The athlete and one parent/guardian will be permitted into the building (no siblings or friends unless they also have a scheduled appointment at the same time). Social distancing and masks will be required because the medical providers will be within the 6 foot proximity of the athlete during the exam. A parent or guardian must be present with the athlete in order for the athlete to be permitted to have the physical performed.

Please note that physicals completed at another location, such as Orthopaedic Associates of Lancaster (OAL), Urgent Care, etc., may require all paperwork to be completed prior to your arrival. Check with the provider prior to your appointment. You will have to enter information for Sections 1-5 and the Risk of Injury Acknowledgment into the ATS system and ONLY provide Section 6 of the CIPPE forms in paper form. No other physical form will be accepted, such as the Dept of Health or the one provided by the doctor’s office. The current CIPPE form is marked with the watermark 2020-21.

Physicals MUST be completed after June 1st of the year for participation in the sportPhysical forms dated earlier than June 1st will not be accepted. Physicals should be turned into the Athletic Trainers at least two weeks prior to the start of the season (Fall Deadline is August 7th, 2020). Athletes are not permitted to practice until the required paperwork is submitted. Physicals may be used for multiple sports within the same school year. If your son or daughter has suffered an injury or illness during school PE, sports, or out of school since the completion of their physical and wants to participate in a sport in an additional season each year, a CIPPE section 8 must be completed to be eligible to try out. If you have seen a doctor, whether on your own accord (NOT RELATED TO SCHOOL), by the nurse, or athletic trainer, this form must be completed.

In addition, if your son or daughter requires any medications to be given at school, please fill this Medication Consent Form out and have it signed by both you as the parent/guardian and a doctor.
