Cedar Crest Athletes, Parents & Guardians,
With winter sports in full swing, the Cedar Crest Athletic Training Staff would like to inform athletes planning to play a spring sport of due dates for physicals. January 22, 2024, is the first day that the Athletic Training Staff can begin accepting physicals for the spring. Forms needed are sections 1-6 of the PIAA physical and the risk of injury form, which should all be completed in ATS. The PIAA Section 7 form is also needed, it should be filled out by the doctor and uploaded into ATS or dropped off at the high school main office. For any athlete that had their physical completed prior to January 22, 2024, (this includes all fall & winter athletes) a PIAA section 8 form must be complete in ATS.

With the first day of spring sports being March 4, 2024, the due date for paperwork will be Monday February 26, 2024. This means ATS will close at 11:59 p.m. on February 26, 2024, and forms will not be able to be entered past this time.Once ATS is closed, we will process paperwork and create cleared to play lists for coaches. ATS will be open again, but any athlete that turns in forms after February 26, 2024, is not guaranteed to be cleared for the first day of tryouts/practice.
Please refer to the Cheat Sheet on the district website for all instructions to complete physical forms. Any new athlete that does not have an ATS account will need to reach out to the Athletic Training Staff to have an ATS account made for them.
